Blue Ridge Hospice has been the exclusive provider of licensed hospice care to people in a five-county area since 1981. A private, non-profit, community-based organization, Blue Ridge Hospice is licensed by the Commonwealth of Virginia and certified by Medicare/Medicaid to provide hospice and home heath services. We are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Included in our services is a free-standing, 8-bed residential center.

Our location near the Blue Ridge Mountains in Virginia offers a wide range of living choices, easy access to Washington, D.C., and all the amenities of the Northern Virginia Area including first-rate schools, cultural activities, and the pleasures of country and/or suburban environments.

Clinical Positions

Hospice and Palliative Medicine Physician – Full Time

Blue Ridge Hospice, well respected, well established not-for-profit hospice serving the Shenandoah Valley for 30 years, and its affiliate, Palliative Medicine Consultants, is seeking to recruit a Hospice/Palliative Care Physician. This individual will practice in collaboration with Hospice home care interdisciplinary teams and perform hospital-based palliative care consultations. The physician will assist in future development of the palliative care program and demonstrate the desire to champion palliative care.

Job Responsibilities:

•  Provide inpatient and outpatient palliative consults.
•  Provide hospice homecare patient visits.
•  Participate in interdisciplinary hospice home care team meetings.
•  Manage Palliative patients’ pain and non-pain symptoms with medication and other   treatment.
•  Evaluation for hospice certification of terminal prognosis of patients, quality of initial plans of   care and comprehensive plans of care, revocations, re-certification of terminal prognosis,   quality of pain an symptom management.
•  Ensure accurate documentation of patient care to support reimbursement for services   provided.
•  Adhere to the practice of confidentiality regarding patients, families, staff and the   organization.
•  Participate in on-call rotation as needed.

Qualifications:

•  Doctor of Medicine or Osteopathy licensed to practice medicine in the state of Virginia.
Board certification in a primary care specialty, Board certified in Hospice and Palliative care   preferred.
•  Current DEA license.
•  Proven track record with interdisciplinary teams, effectively establishing goals of care and   building collaborative physician/patient/family relationships in addition to a comprehensive   knowledge of hospice and palliative care, pain management and medical ethics preferred.
• Previous hospice and/or palliative care experience preferred.


RN/LPN’s-ICC- PRN (7pm to 7am & 7am to 7pm) – Winchester 

Rotating weekends and holidays 
RN/LPN on a PRN as needed basis for our 8-bed hospice inpatient unit based in Winchester, VA Current RN/LPN state license required
Position requires minimum of 2 years current acute care experience
Previous hospice experienced preferred
Must have excellent assessment and critical thinking skills
Ability to work within an interdisciplinary team setting
Must be familiar with general use and functions of the computer


Referral  Coordinator – Full time RN/SW– Winchester

As a member of the Assessment Department, the Referral Coordinator will provide oversight, supervision and coordination of the intake and referral process for prospective hospice patients.  
Works collaboratively with other members of the assessment team to assure prompt response to all referral sources and timely, accurate and comprehensive documentation of each step of the referral process.  
Schedules appointments for assessments and informational visits
Develops and maintains excellent relationships with all referral sources, including physicians, hospitals, facilities, patients, caregivers and other vendors and  community resources
Must have current Virginia licensure as an RN or a BSW for Social worker
Previous hospice experience preferred
Must possess excellent organizational, critical thinking, problem solving, communication and customer service skills
Must be able to multi-task
Must be proficient in Microsoft office and use of an electronic medical record and in use of other clinical data bases.
Reports to the Clinical Manager of Admissions

LPN - Palliative Medicine Consultants – Part Time Position (24 hours)

Palliative Medicine Consultants, an affiliate of Blue Ridge Hospice, is seeking to recruit an LPN to assist our interdisciplinary team in our physician practice setting.

Job Responsibilities:
Provide nursing support to our palliative team. 
Delivers care within the scope of nursing practice and in accordance with accepted standards of practice.
Manage Palliative patients’ charts/records inpatient and outpatient.
Ensure accurate documentation of patient contact and follow up.
Adhere to the practice of confidentiality
Assist with administrative duties.
Qualifications:
Current unrestricted LPN state license required.
Knowledge of Palliative Care preferred.
Must have excellent assessment and problem solving skills.
Ability to work with an interdisciplinary team.
Must be familiar with general use and functions of a computer.  Including Microsoft Excel, Microsoft Word and general knowledge of Electronic Medical Record Applications.


Reliable transportation required. Background check and drug testing required at time of hire.
