Blue Ridge Hospice has been the exclusive provider of licensed hospice care to people in a eight-county area since 1981. A private, non-profit, community-based organization, Blue Ridge Hospice is licensed by the Commonwealth of Virginia and certified by Medicare/Medicaid to provide hospice and home heath services. We are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Included in our services is a free-standing, 8-bed residential center.Our location near the Blue Ridge Mountains in Virginia offers a wide range of living choices, easy access to Washington, D.C., and all the amenities of the Northern Virginia Area including first-rate schools, cultural activities, and the pleasures of country and/or suburban environments.

 The following employment opportunities are available: 

RN On Call - (Full Time - 40 hours)

  • • Provides telephone triage symptom management and focused home visits 
  • • Requires the ability to drive throughout service area
  • • Willingness to work either evening or night shift & rotating weekends
  • • Current RN state license required with a minimum of 2 years acute care experience
  • • Previous hospice experienced preferred
  • • Must have excellent assessment and problem solving skills
  • • Ability to work within an interdisciplinary team setting
  • • Must be familiar with general use and functions of the computer

 

Assessment RN - (Full Time - 40 hours)

  • • Performs the assessment function and derives nursing diagnoses based on assessment data
  • • Coordinates assessment visits with Family Support Services
  • • Documents in electronic medical record
  • • Provides Plan of Care to the team members utilizing voice care system
  • • May act as liaison with hospital, monitorying patients to ensure appropriate care
  • • Shares responsibility for weekend rotation
  • • Current RN state license required with a minimum of 2 years acute care experience
  • • Previous hospice experienced preferred
  • • Must have excellent assessment and problem solving skills
  • • Ability to work within an interdisciplinary team setting
  • • Must be familiar with general use and functions of the computer

 

RN Case Manager - (Full Time - 40 hours)

  • • Winchester/Frederick County and surrounding areas
  • • Position involves travel withing a specified local territory to patient homes to provide direct patient care and education to Hospice patients and their caregivers
  • • Current RN state license required with a minimum of 2 years acute care experience
  • • Previous hospice experienced preferred
  • • Must have excellent assessment and problem solving skills
  • • Ability to work within an interdisciplinary team setting
  • • Must be familiar with general use and functions of the computer

 

Medical Social Worker - (2 Full Time positions - 32  & 40 hours and PRN)

  • • Responsible for psychosocial assessments & provision of medical social services to patients & families
  • • Must be able to drive throughout our service area - 32 hour position
  • • Requires frequent weekend On Call coverage - 32 hour position
  • • Candidates should have prior short term individual and crisis counseling
  • • Conducts safety assessments and assists patients in completing applications for federal assistance programs
  • • Previous hospice experienced preferred
  • • Must have excellent assessment and problem solving skills
  • • Must be flexible with daily duties, have a positive outlook, be self-motivated and a team player
  • • Requires bachelor's degree in social work, MSW preferred
  • • Requires 2 years experience in case management and supportive counseling
  • • Ability to work within an interdisciplinary team setting
  • • Must be familiar with general use and functions of the computer
