Hospice Physician – Winchester – Full Time

  • Job Responsibilities:
  • Provides hospice homecare patient visits
  • Participates in interdisciplinary hospice home care team meetings
  • Evaluation for hospice certification of terminal prognosis of patients, quality of initial plans of care and comprehensive plans of care, revocations, re-certification of terminal prognosis, quality of pain and symptom management
  • Ensures accurate documentation of patient care to support reimbursement for services provided
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization
  • Participate in on-call rotation as needed
  • The physician has the responsibility for the medical care and treatment of patients rendered by the hospice interdisciplinary team (IDT) and shall consult and cooperate with the patient’s attending physicians
  • The physician provides direction and guidance to the staff and volunteers of the hospice IDT to assure the maintenance of quality standards of care for patients and families
  • Actively participates in the clinical education and training for hospice patient care, admissions, and administrative personnel
  • Qualifications:
  • Doctor of Medicine or Osteopathy licensed to practice medicine in the state of Virginia
  • Board certification in a primary care specialty, Board certified, or eligible, in Hospice and Palliative care
  • Unrestricted narcotic DEA license
  • Proven track record with interdisciplinary teams, effectively establishing goals of care and building collaborative physician/patient/family relationships in addition to knowledge of hospice and palliative care, pain management and medical ethics preferred
  • Intermediate computer knowledge
  • Previous hospice and/or palliative care experience preferred
  • Knowledge of the principles and practice of primary medical care

Nurse Practitioner Position – (Full time)

  • Collaborates with the hospice home care and palliative care interdisciplinary teams
  • Provide palliative care and hospice homecare patient visits
  • Rotating on-call duties with physicians for week nights and weekends
  • Provides support and education to hospice interdisciplinary team
  • Must hold a Virginia NP license
  • Prescriptive ability in the state of Virginia with a current DEA certificate
  • Minimum of 3 years of experience, preferably in palliative care, oncology, and/or hospice
  • Must have excellent assessment, diagnosis and treatment skills
  • Excellent communication skills
  • Self-directed, motivated, and highly organized

Manager, Quality Assurance/Performance Improvement & Education – Full Time

  • Develops, plans, implements and evaluates Blue Ridge Hospice’s Performance Improvement Plan and performance improvement initiatives.
  • Analyzes trends, and reports all performance improvement and quality data utilizing continuous quality improvement principles and methodologies on a monthly, quarterly, and annual basis.
  • Communicates identified areas of concern/opportunities for improvement to the Chief Clinical Officer and other leadership staff.
  • Chairs the Quality Committee and completes reports for the Board of Directors.
  • Maintains knowledge on current infection control standards and practices.
  • Ensures a timely response to all levels of Medicare request for medical record documentation related to Additional Development Requests (ADRs) and manages all levels of appeals including:  Redeterminations, Reconsiderations and Administrative Law Judge.  Writes or edits all letters and ensures a final version is delivered to the Business Office prior to the due date.
  • Coordinates and leads the organization’s accreditation process.
  • Collaborates with clinical leadership to ensure employee orientation, initial competency assessment, annual competency training and in-service educational programs.
  • Establishes and chairs the Nursing Education Committee.
  • Develops a monthly calendar of educational offerings.
  • Graduate of an accredited school of nursing; BSN required; and Master’s Degree preferred
  • Three-five years of nursing experience, including three years of progressive nursing leadership experience
  • Must have a comprehensive knowledge of nursing practice, applicable regulatory requirements and standards of care related to hospice
  • Must have strong organizational skills with proficiency in Microsoft Office, data collection and analysis, excellent communication skills, and the ability to work with staff from a variety of disciplines in multiple settings

RN On Call – (Full Time – 32 hours)

  • Current RN state license required with a minimum of 2 years acute care experience
  • Previous hospice experienced preferred
  • Must have excellent assessment and problem solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
  • Position provides telephone triage symptom management and focused in home visits
  • Requires ability to drive throughout Blue Ridge Hospice service area
  • Willingness to work either evening or night shift and rotating weekends

Certified Nursing Assistant (CNA)  – Home Care –Full Time

  • Warren County, Luray, Frederick County areas
  • Position involves travel, within a specified local territory, to patient homes to provide direct patient care and education to Hospice patients and their caregivers.
  • Current CNA state license required.
  • Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient. May involve assistance with bathing, ambulatory needs, exercises, assists with self-administered medication and assists with eating.
  • Takes and records patient’s temperature, blood pressure, pulse and respirations. Reports any changes in the patient’s condition or family situation immediately to the primary nurse.
  • Attends at least twelve (12) hours of continuing education programs or approved audiovisuals per calendar year
  • Previous hospice experienced preferred
  • Must have excellent assessment and problem solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer

LPN, Inpatient Care Center – PRN

  • 12 hours shifts
  • Rotating weekends, holidays, day and nights
  • LPN needed for our 8-bed hospice inpatient unit based in Winchester, VA
  • Current LPN state license required
  • Position requires minimum of 2 years current acute care experience
  • Must have excellent assessment and problem solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
  • Previous hospice experience preferred

RN – Float– (Full Time)

  • Current RN state license required
  • Position requires minimum of 2 years current acute care experience
  • Previous hospice experienced preferred
  • Must have excellent assessment and problem solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
